Show Report Form

Double Shows require a Report Form for each show.
You have 30 Days to file your Report Form, from the date of the Show EXCLUDING December reports.
All December Report Forms are Due by December 31, with NO EXCEPTIONS.
Juniors who turn in a Report Form will qualify for the Phoenix Meet Showmanship Pins and Junior Exhibition Achievement Pins.
When you file your Report Form you will earn your Open or Junior Points for Awards
